To be eligible for the Ontario Health Funding Opportunity for FNIMUI Lower-Limb Preservation, you must be:
- A First Nations, Inuit, Métis, or Urban Indigenous (FNIMUI) community or organization
- Located in Ontario
- Focused on supporting people with diabetes or vascular issues
Who can apply for the funding?
You can apply if you:
- Are Indigenous-led or governed
- Submit a complete Expression of Interest (EOI) form
- Work with partners like clinics, hospitals, or foot care providers
- Plan to track your results and improve care for your community
What are the conditions?
To qualify for funding, your project must:
- Focus on one or more of these goals:
- Connected team care
- Consistent wound treatment
- Early detection and prevention
- Be co-designed with patients and families
- Consider digital health options like remote checkups
- Include a budget plan and a way to measure impact (e.g. wound healing rates, fewer amputations)
How much funding is available?
The Ontario Health Funding Opportunity for FNIMUI Lower-Limb Preservation provides:
- Up to $500,000 per year for each selected project
- 3 years of support, starting in 2025/26
- Possible renewal after 3 years
- Funding can cover:
- Staffing (e.g. foot care nurses, patient navigators)
- Supplies and equipment (e.g. wound kits, screening tools)
- Training programs or health workshops
- Community education and digital tools

- Application Due: June 27, 2025
- Funding Starts: 2025/26
- Support Length: 3 years (with renewal option)
